Facility staff may collect observations across several shifts and assemble them into a dated assessment. CMS maintains the Resident Assessment Instrument manual for nursing homes. A synthetic example can test whether software distinguishes notes in the relevant window from notes added later. CMS Resident Assessment Instrument manual

Create a fictional three-day window with original observations about an invented training activity. Add a note outside the window and a correction that refers to an earlier observation. Ask which evidence the model can use and whether the revised assessment was reviewed. Score timing and review status separately.

Keep resident files, room records, call logs and health details out. Use your own forms and have staff validate the example. Leave real assessment coding and clinical decisions outside the task.

Describe a fictional assessment evidence sample

Original Fiund exercise

Event time, entry time and amendment review affect an assessment evidence packet

An original three-day assessment fixture concerns a harmless training-room sorting activity. It contains no resident observation, clinical coding, call log or health detail.

Fictional records supplied to the evaluator
RecordSourceWhat it records
NA1Practice windowAssessment A covers event days 1–3. Training observation O1 event day 2, entered day 2, label COMPLETE.
NA2Later sourcesO2 event day 4, entered day 4. Correction C1 entered day 5 explicitly changes O1's day-2 label to PARTIAL; retains its original event date.
NA3Assessment versionA-v1 reviewed on day 3. A-v2 incorporates C1 on day 5 but review signature blank. Exercise permits linked corrections to in-window events to be flagged separately.

Question for the model

Which events belong to the stated window, and what review state applies to the revised assessment?

Show the reference answer and checks

NA1's O1 occurred within days 1–3. NA2's O2 occurred on day 4 and falls outside that event window. C1 is a later correction to in-window O1, so preserve its original event date and later entry date under the stated exercise rule. NA3's A-v2 includes that correction but lacks review approval; A-v1's earlier signature does not automatically approve the revision. Report the source and review distinctions without performing actual assessment coding.

  • Using entry time instead of event time to include or exclude observations.
  • Treating an earlier assessment signature as approval of a later amended version.

Fields to preserve

Window and event date
Defines temporal eligibility.
Entry date and correction parent
Preserves later amendments.
Assessment version and review signature
Separates assembled from approved records.

Test another case

Add a day-5 entry documenting a new day-3 event. Require its event-window fit and late-entry status both to be visible under the original rule.

Use original nonclinical forms validated by staff. Exclude resident files and room records; real coding and clinical decisions remain outside the task.

Discuss a contribution

Describe the sources you control, the task they could support and what has already been checked. Use the existing-data assessment for an archive or the collection brief for a new recording project. The contributor agreement defines Fiund’s licensing authority and compensation. A guide or fictional exercise does not establish an accepted collection or a sale.
