An employer’s contribution record can support one part of a benefits review without proving that a participant is eligible. DOL plan information and Form 5500 materials give context for benefit plan records and reporting. A fund can test whether an assistant assembles the right mock file for authorized review. DOL plan information · DOL Form 5500 instructions
Invent two contribution reports under the same sponsor alias with different coverage months. Include a review acknowledgment and a missing plan-specific record. Ask which month the evidence describes and what remains unresolved. Include a record for a different benefit program with a similar name. Score identification and uncertainty, with eligibility left to authorized staff.
Use no employee, dependent, health, payroll, claim, or real contribution information. Keep the fictional plan rules clearly marked as practice material.

Original Fiund exercise
A contribution acknowledgment identifies a month without determining benefit eligibility
A fictional sponsor and two invented benefit programs have original practice records. No employee, dependent, payroll or health information appears.
| Record | Source | What it records |
|---|---|---|
| HF1 | Contribution reports | Sponsor S, program H: report J covers January; report F covers February. Each has its own period reference. |
| HF2 | Review acknowledgment | Acknowledgment AJ confirms receipt of S/H/J only. Plan-specific service confirmation requested but not included. |
| HF3 | Similar program file | Sponsor S, program H2: February acknowledgment complete. No evidence that H2 records apply to H. |
Question for the model
Which H contribution period has a receipt acknowledgment, and does the packet establish a participant's benefit eligibility?
Show the reference answer and checks
HF2 explicitly acknowledges January report J for program H. HF1's February H report lacks a corresponding acknowledgment, and HF3 belongs to separate program H2. The requested plan-specific service confirmation is absent, and there are no participant records in this fixture. Assemble those period and program findings for authorized review; receipt of a contribution report alone does not resolve the eligibility question posed by the packet.
- Treating a sponsor-level contribution acknowledgment as a participant-level eligibility determination.
- Borrowing a similarly named program's acknowledgment to close a different program's missing record.
Fields to preserve
- Sponsor, program and coverage month
- Defines the record's scope.
- Report and acknowledgment link
- Documents receipt of a particular submission.
- Requested plan evidence and review state
- Preserves the unresolved part of the file.
Test another case
Add February's H acknowledgment while leaving service confirmation absent. Require the receipt gap to close without converting it into an eligibility outcome.
Use independently authored fictional plan records. Exclude employee and dependent information; authorized plan staff retain eligibility interpretation and decisions.
Discuss a contribution
Describe the sources you control, the task they could support and what has already been checked. Use the existing-data assessment for an archive or the collection brief for a new recording project. The contributor agreement defines Fiund’s licensing authority and compensation. A guide or fictional exercise does not establish an accepted collection or a sale.
