Chiropractic visit and exam documentation data for AI

A template can carry an observation from an earlier visit into a later draft.

A template can carry an observation from an earlier visit into a later draft. CMS publishes a chiropractic documentation checklist. A practice can make an independently written example that evaluates which observation belongs to which visit. CMS chiropractic documentation checklist

Create two fictional visit records and one reference sheet with a dummy observation that was entered only on the first date. The later draft says a check was scheduled. Ask the model which note records an observation and whether the later visit has matching evidence. Have a reviewer score visit date and observed versus planned status.

Use fictional forms and no real examinations, charts, images or patient audio. Keep diagnosis, manipulation techniques, coding and coverage decisions outside the sample.

Describe a fictional chiropractic records sample

Original Fiund exercise

A carried-forward template line does not create a later visit observation

An independently written administrative example uses dummy visit IDs and abstract observation labels. It contains no examination, technique, patient chart, image or audio.

Fictional records supplied to the evaluator
RecordSourceWhat it records
CX1Visit V1 noteDay 2: abstract observation O recorded as directly observed. Source belongs to V1 only.
CX2Visit V2 draftDay 9: template carries O text from V1. Separate field says check scheduled, completion blank; draft not signed.
CX3Reference sheetExplicit provenance map links carried text to V1. No V2 observation source supplied.

Question for the model

Which visit has an actual recorded observation in this fixture, and can the V2 draft be summarized as a repeated completed check?

Show the reference answer and checks

CX1 records O for V1. CX2's copied text and scheduled-check field do not document a completed V2 observation, and CX3 explicitly identifies V1 as the text's source. Report V2's planned check and missing completed evidence separately instead of treating carried-forward language as a fresh finding. The exercise addresses provenance only and provides no basis for diagnosis, clinical technique, coding or coverage decisions.

  • Treating repeated template text as a new observation.
  • Converting a scheduled check or unsigned draft into a completed visit finding.

Fields to preserve

Visit and source date
Defines when the observation occurred.
Copied-text parent and draft state
Preserves provenance.
Planned check and completion evidence
Makes the missing event explicit.

Test another case

Add a signed V2 note that explicitly says the check was not completed. Require the documentation state to resolve without inventing a negative clinical result.

Use original fictional forms only. Exclude patient data and real exams; qualified staff retain medical, coding and coverage review.

Discuss a contribution

Describe the sources you control, the task they could support and what has already been checked. Use the existing-data assessment for an archive or the collection brief for a new recording project. The contributor agreement defines Fiund’s licensing authority and compensation. A guide or fictional exercise does not establish an accepted collection or a sale.
