Record a fictional documentation task
Use adult actors and a training room. Create a fabricated visit scenario and capture an approved short interaction. Have the reviewer label what was directly observed, what was reported by the actor, and what must remain unknown. Include a case where the assistant should ask for clarification. Evaluate the accuracy of documentation, source attribution and the handoff to a human. Keep diagnosis, care decisions and recommendations outside this exercise.
Start without patient or household information
Get written permission from the organization, venue and people involved. Keep real health information, patient faces or voices, actual addresses and live systems outside the simulation. If someone proposes actual records, route that separately through the privacy lead and qualified counsel. HH2 supports only that the actor reported putting a blue card inside yesterday; it does not show the contents or verify that earlier action. HH3 misattributes that report as a reviewer observation and adds unsupported overall completion. Correct the source attribution and retain the unknown contents without making any diagnosis, care decision or clinical interpretation.

Test another case
Add a separate visible folder-opening clip showing a red card. Require the direct observation and earlier blue-card report to remain distinct with the discrepancy flagged.
